Nurse’s Office

School nurses and nurse-teachers are assigned to each building to implement the Health Services Program through teaching, counseling and administering first aid. They also conduct NY required screenings for height, weight, vision, hearing and scoliosis. Nurses are available to students and families for health information, advice and medical questions as well as to implement student medication schedules during the school day.  Nurse’s offices are open whenever school is in session and students are always welcome.

Please note that physical exams are required for all NEW entrants and students entering grades K, 2, 4, 7 and 10. To be accepted for school entrance, physicals need to have been completed after September 1 of the previous school year. Physicals will be provided by the school physician after that date for students who do not have a physical report on file with the school nurse. Information regarding physicals for fall sports

Physical Examination for Fall Sports

  • A physical examination is valid for 2017-18 High School fall sports if it was performed on or after August 1, 2016.
  • A physical examination is valid for 2017-18 Middle School fall sports if it was performed on or after September 1, 2016.

If a student’s primary medical provider has not performed a physical exam within the required time frame, physical examinations for High School and Middle School athletes are available at the High School Nurse’s Office between 8:00 a.m. and 10:00 a.m. on Thursday, August 10 and Tuesday, August 15.

Athletes must check with the Nurse’s Office to see if a physical is required.

Parent Permission/Sports Update Form

The parent permission/sports update form must be approved in person at the Nurse’s Office.

High School: The athlete must bring the completed form to the Nurse’s Office between 7:00 a.m. and 11:30 a.m. on August 9, 10, 11, 14, 15, or 16.

Middle School: The athlete must bring the completed form to the Nurse’s Office between 8:00 a.m. and 12:00 p.m. on Wednesday, August 30 or Thursday, August 31.
