*Please note: Children must be 5 years old on or before December 1, 2013.

Directions for Kindergarten Registration ONLY

1. Download & print all forms below.
   
   
2. Fill out all applicable information on the forms (print & use ink).
   
   
3. Parent/Guardian must sign the registration form at the bottom of page 2 & the Home Language Questionnaire.
   
   
4. Please be sure to list the appropriate baby-sitter/child-care information on the Transportation form.
