FREE & REDUCED MEAL APPLICATION FORM

GETTING MEAL BENEFITS

 

Children need healthy meals to learn.  Monroe-Woodbury offers its students a choice of nutritionally balanced meals every school day. 

 

Children from households that meet federal income guidelines are eligible for free meals or reduced price meals.  Reduced price meals cost each eligible student $.25 for lunch and $.25 for breakfast.  Foster children are eligible for meal benefits.  If you believe you might qualify, we encourage you to apply for meal benefits.  All children in the qualifying household receive the benefit of the free or reduced meals.  All information will be kept confidential.  Other parents, students or unauthorized school personnel will not be aware that your family is participating in this free or reduced meal program.

 

 

HOW TO APPLY:

 

To apply for free or reduced price meals, you may either:

  • Submit a Direct Certification letter received from the NYS Office of Temporary and Disability Assistance, OR
  • Complete one application for the household (proceed to link below to print application)

Please refer to the guidelines when completing the application.  We cannot approve an application that is not complete, so be sure to fill out all required information and sign the application.  For help completing the application, call the Food Service Office at 845-460-6110.

 

If your child was on the free or reduced program last year, you must reapply before September 30, 2012.

 

Applications can be given to the cafeteria manager, nurse, or school office.  They can also be faxed to 845-460-6061 or mailed to:

You will be notified by mail when your application is approved or denied.

 

If your child is accepted into the free and reduced program, please note that a free or reduced meal is the complete meal as listed on the menu.  You are responsible for incomplete meals or ala carte purchases.   PLEASE EXPLAIN THIS TO YOUR CHILD.   You are responsible for any charges up until your acceptance into the program.
