APPR Observation Forms & Info

The APPR form is linked below. It is a single file that includes the following forms for 2012-2013:

  • Lesson Plan Form (Pre-Observation Conference Form)
  • Teacher Observation Form
  • Professional Responsibilities Form
  • Principal Walkthrough Form
  • Monroe-Woodbury 0-60 Conversion Chart

**This is a FILLABLE FORM.

Please use FIREFOX version 19 to download the APPR Form and ADOBE READER to fill it out. Directions are below.


To check your version of Firefox:

  • Double click on Firefox to open
  • In the top left hand corner of the menu, click on FIREFOX and drop down to ABOUT FIREFOX
  • It should say 19.0 under Firefox

To download the APPR form:

  • Open Firefox version 19
  • Click on the 2012-2013 APPR Forms link below
  • Click on OPEN WITH DIFFERENT VIEWER

Open With

  •  Click the OPEN WITH button
  •  If ADOBE READER is not selected, click on the drop down and choose OTHER and then choose ADOBE READER from applications

Open With

  • Check off the DO THIS AUTOMATICALLY FOR FILES LIKE THIS FROM NOW ON box
  • Click OK.

The file will open with Adobe Reader. You may save it and fill it out at a later date if you wish but you must remember to complete the form in ADOBE READER.

To open the file with ADOBE READER after saving it from the website:

  • Go into your APPLICATIONS folder
  • Double click on ADOBE READER
  • Go to FILE----->OPEN
  • Navigate to the location you saved your APPR form in and click on your file

***Please save the file as:

"YOUR LAST NAME"_APPR.pdf
