New Procedure for Working Papers due to COVID-19 closures in NY

During the period of school closures due to COVID-19, Monroe Woodbury will be issuing working cards using the following alternative process.

1. Parents/guardians and minors complete the appropriate application form (see below) and email it to [email protected]. NOTE: Evidence of a physical examination from your doctor within the last 12 months must be provided with the application.

Fillable PDF versions of the AT-17 Application for Employment Certificate are available on the school’s website as well as the NYSED’s Employment of Minors website. These forms allow digital signatures.

2. The Certificating Officer will then process the working card and mail it to the applicant. The minor should sign the card upon receipt. The requirement that the minor sign the card in the presence of the certificating officer is temporarily waived for the duration of New York’s COVID-19 school closures.

New York Working Papers are NOT available through the high school guidance department while schools are closed due to COVID-19.

  • Summer hours: working papers are prepared Monday-Friday from 9:00 a.m. – 12:00 p.m. in the House A Office
    • Please call (845) 460-7000 x 7012 if you have questions or need assistance.
  • School year hours: applications must be dropped off prior to the start of the school day or during lunch periods.
  • Please allow two days for processing working papers.

  • Blue working papers – For students between the ages of 14 and 15
  • Green working papers – For students between the ages of 16 and 17

Students must bring the completed 2 part Application for NY State Working Papers and all required documents with them to apply. Please use the links below to download the forms. Then be sure to read the Working Papers Instruction below carefully!

Please read these instructions carefully before completing the Application

  • Failure to complete form requirements will delay receipt of Working Card
  • Use ONLY blue or black ink! No pencils!
  • Do NOT fold the application
  • Social Security Cards ARE NO LONGER NEEDED

Students are responsible for completing the following:

Physical Fitness Certification – NY State Form AT-16

  • Student must have had a physical examination within the past 12 months
  • Student is to complete the top section of the Physical Fitness Certification Form:
    • student name, complete address – including house number, street name, city, state and zip code
    • date of birth and gender must be completed

Part A or Part B needs to be completed by either the student’s private physician or the Monroe-Woodbury School Nurse.

1- If using a private physician, the form MUST include the date of the physical, the physician’s signature AND the physician’s stamp.
2- If the School Nurse is completing the form, the student must have an updated physical on file in the nurse’s office.
3- The student is responsible for requesting that Part A or Part B be completed by visiting the Nurse’s office and arranging for the form to be done.

Part 1 – Parental Consent

  • PLEASE COMPLETE PART 1 ONLY with the student’s name, age, complete home address (including house number, street name, city, state and zip code) and the parent’s signature.
  • Birth Certificates are necessary ONLY if the student IS NOT a student from the M-W school district.

When the form is fully completed:

The application can be dropped off and picked up in the House A Office prior to the start of the school day or during lunch periods ONLY!

Here are a few tips…

  1. You must have had a physical in the past 12 months to apply for working papers.
  2. The physical can be on file in the nurse’s office or done with your private doctor.
  3. If your doctor did the physical, s/he must stamp and sign the application form.
  4. The student MUST be present when picking up working papers or they will not be released.

It is important for students to keep their documents current to remain employed.
