Sign up for MS fall sports teams in August

on April 1, 2019

The Board of Education supports a program of interscholastic athletics at the Modified level for students in grades 7 and 8. The New York State Education Department does not permit students in Grade 6 to participate in interscholastic sports. Sixth grade students are permitted to participate in intramural sports. Plan ahead to be sure requirements are completed by the August deadlines. Visit the Athletics page on the district website for additional information about Interscholastic Sports.

Monroe-Woodbury Middle School will offer the following Interscholastic Sport programs at the Modified level for 7th and 8th grade students in the Fall of 2019:

  • Cross Country – (Boys and Girls)
  • Soccer – (Boys and Girls)
  • Cheerleading
  • Volleyball – Girls
  • Football – Modified “B”

All Middle School sports will start on Wednesday, September 4 (the first day of school).

Requirements to sign up

To participate in a Fall Sport team, students must first receive clearance from the Middle School Nurses’ Office.  Three requirements must be completed. All required forms are available for download on the district website Forms page.

1 – Physical examination

Students whose physical exam was done on or after September 1, 2018 and is already on file in the Nurses’ Office do not need another physical examination to participate in a fall sport.  If students need a physical, they will be available at the High School Nurses’ office on Thursday, August 15 and Tuesday, August 20 between the hours of 8:00 a.m. and 9:00 a.m.  Physical examinations may also be conducted by a family physician, in which case students will need an M-W Medical Examination Form to be filled out by the physician and delivered to the Middle School Nurses’ Office for clearance.

2 – Sports Update

Updates must be done no sooner than thirty days before the opening day of each sport season. Each athlete must appear in person at the Middle School Nurses’ Office for an update, which will be conducted from 8:00 a.m. to noon on Wednesday, August 28 and Thursday, August 29, 2019.

Medication only as noted below

Please note that students who have a history of asthma, food allergies or bee sting allergies are required to have their medication at the Sports Update.  In order to use medication in school or at sports functions, students must have on file at the Nurses’ Office:

    1. a physician’s order for the medication
    2. written permission from a parent or guardian for the medication to be given
    3. the medication in its original container with the student’s name on it

These items must be brought to the Nurses’ Office at the time of the Sports Update.

3 – Parent/Guardian Permission/Update

The Parent/Guardian Permission form can be downloaded along with the Authorization for Medication form and the Physical Examination form from the Forms page on the district website.
